To lead the way in treating chronic illness, we’re letting our patients get on with their lives.

If you had a chronic illness, would you rather go to regular check-ups at hospital or get all the care you need in the comfort of your own home? We think we know the answer and, thanks to a new innovation by Sheffield PCT, this second option is quickly becoming reality.

The Sheffield Telehealth Team is conducting a project to use remote access technology to monitor patients with Chronic Obstructive Pulmonary Disease, or COPD, at home. The system enables the transmission of vital signs, including blood pressure, temperature and weight, to a central monitor in the COPD office and the respiratory nurse’s office via the patient’s telephone line.

The result? Clinicians are able to use the physiological data collected as well as other lifestyle data to ensure that a patient’s care plan is right for them. Nurses can monitor their patients without the necessity of making regular time-consuming home visits. Patients become more aware of their condition and can take control of it, giving them more freedom in their everyday life. Hospitals can save money by cutting down unnecessary hospital attendance and admission.

It’s a win-win piece of technology, and with the potential to be translated to the provision of healthcare services for other conditions, the opportunities are endless.
